AEB Benefits, LLC, doing business as Level Health Plans ("Level Health," "we," "us," or "our"), provides this policy to explain how we collect, use, and share information through our public website at levelhealthplans.com, including business inquiries, provider submissions, plan-analysis requests, event registrations, and our text messaging program.
This policy covers our public website activities and the mobile information described in the SMS section below. Health-plan records, claims, enrollment, and member services may be governed by separate privacy notices, including an applicable health plan’s HIPAA Notice of Privacy Practices. This website policy does not replace those notices. Contact your plan or the member service identified on your ID card for information about those practices.
Please do not submit medical records, diagnoses, Social Security numbers, claims details, or other sensitive personal or health information through general website inquiry forms or ordinary email. Contact us for guidance on an appropriate channel when such information is necessary.
Information we collect
Information you provide. Depending on the form or request, this may include your name, work email, telephone number, company or practice name, job title, location, website, employee or provider count, provider type and specialties, timing preferences, event registration details, and any message or other information you choose to provide.
Information collected through website use. Our hosting and technology providers may process IP addresses, browser and device information, referring pages, pages visited, interactions, timestamps, and cookie identifiers. Form submissions may include the page address and title and an existing HubSpot cookie identifier, allowing a submission to be associated with prior website activity.
How we use information
We use information to respond to inquiries, discuss plan options and business opportunities, evaluate provider-network requests, arrange plan analyses, register and communicate with event attendees, and follow up about our services and events. We also use information to understand website usage, improve our website and communications, maintain security, troubleshoot problems, and address legal obligations and disputes.
Text messaging and mobile privacy
When you opt in to the Level Health Plans Service & Support text messaging program, we collect and use your mobile number, message content, delivery information, and opt-in and opt-out records to provide requested support and service updates, deliver messages, maintain consent records, and honor your communication preferences. Messages may be handled by our team or automated systems, including Capacity. Our SMS Terms & Conditions explain the program.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent are not sold, rented, or shared with third parties or affiliates for their own use. These restrictions apply to the partner and sponsor sharing described elsewhere in this policy.
We may provide mobile information and message records to service providers, including Capacity and messaging delivery providers, solely to operate the program, provide support, deliver messages, and manage opt-in and opt-out requests on our behalf. This does not authorize those providers to use that information for their own marketing.
We retain message and consent records as needed to provide the program, document permissions, honor opt-outs, and meet applicable legal requirements. Opt-out records may be retained to prevent further messages.
Reply STOP to unsubscribe or HELP for help. You may also contact info@levelhealthplans.com. Message frequency varies. Message and data rates may apply. We may send one final opt-out confirmation; further program messages require a new opt-in. Website inquiry or event registration alone does not constitute consent to recurring texts.
SMS is not a secure channel for sensitive health information. Do not text medical records, diagnoses, Social Security numbers, claims details, or other sensitive information. This SMS notice does not replace an applicable health plan’s HIPAA Notice of Privacy Practices.
Your choices and privacy requests
You may unsubscribe from marketing emails using the unsubscribe link where provided, or contact info@levelhealthplans.com about our communications. We may still send non-marketing emails needed to respond to a request or administer an event you registered for. SMS opt-outs are handled as described in the text messaging section above. For messages sent independently by a partner or sponsor, use their unsubscribe process or contact them directly.
You may also email us to ask about your information, request access, correction, or deletion, or raise a concern about sharing. Depending on where you live and which laws apply, you may have additional privacy rights. We will assess requests under applicable law and may need to verify your identity. Some information may need to be retained for legal or other permitted purposes. Do not include sensitive health information or identity documents in your initial email.
Retention and security
Retention depends on the purpose for which information was collected, our ongoing relationship with you, operational needs, and applicable legal, accounting, and dispute-resolution requirements. Different types of information may be retained for different periods.
We use safeguards intended to protect information, but no internet transmission or storage system can be guaranteed completely secure. Please take care when choosing what information to send through the website.
Other websites and member services
Our website links to third-party websites and services, including member portals and care resources. Their collection and use of information are governed by their own privacy notices. Review those notices before providing information. A link on our website does not make this policy apply to another provider’s service.
Children’s privacy
Our public website’s business inquiry and event registration features are intended for adults, not children under 13. We do not knowingly collect personal information from children under 13 through these features. If you believe a child has provided information, contact us so we can review and address it. Information about dependent health-plan members is subject to the applicable plan or service privacy notice.
Changes to this policy
We may update this policy as our website or practices change. Updates will appear on this page with a revised date. We will provide additional notice when required by applicable law.
