Official Rules and Conditions for the Level Health Yeti Cooler Giveaway

 1. Sponsor

This giveaway is sponsored by Level Health, referred to as “Sponsor” throughout these rules.

 2. Eligibility

The giveaway is open to legal residents of Missouri who are 18 years of age or older at the time of entry. Employees of Sponsor, its affiliates, subsidiaries, advertising, promotion agencies, and their immediate family members (spouses, parents, siblings, and children) or those living in the same household are not eligible to enter or win. Void where prohibited or restricted by law.

 3. Entry Period

The giveaway begins on 9/23/2024 and ends on 9/27/2024 at 11:59 PM Central Time. Sponsor’s computer is the official time-keeping device for the giveaway.

 4. How to Enter

To enter, participants must submit their basic contact information (name, email, phone number) through the online form provided by the QR code link during the entry period. Limit one (1) entry per person. Duplicate entries will be disqualified.

 5. Prize

One (1) Yeti Cooler will be awarded to the winner. The approximate retail value (ARV) of the prize is $200. No substitution, transfer, or cash equivalent of the prize is permitted except at Sponsor’s sole discretion. Sponsor reserves the right to substitute a prize of equal or greater value.

 6. Odds

Odds of winning depend on the number of eligible entries received.

 7. Winner Selection and Notification

The winner will be randomly selected from all eligible entries within30 days after the giveaway ends. The potential winner will be notified via the contact information provided. If the winner cannot be contacted, does not respond within  7 days, or is found ineligible, an alternate winner may be selected.

 8. Publicity

By entering, except where prohibited, you grant Sponsor permission to use your name, likeness, and entry for promotional purposes without additional compensation or consent.

 9. Liability Release

By entering the giveaway, you agree to release and hold harmless Sponsor and its subsidiaries, affiliates, and related companies, and their respective officers, directors, employees, and agents from any and all liability or any injury, loss, or damage of any kind arising from or in connection with your participation in the giveaway or the acceptance, possession, or use of the prize.

 10. Disputes

This giveaway is governed by the laws of the State of Missouri, without regard to its conflict of law provisions. Any legal action or claim arising out of or in connection with the giveaway shall be brought exclusively in the state or federal courts located in St. Louis County. By entering, you agree to submit to the jurisdiction of such courts and waive any objection to the venue or jurisdiction in those courts.

 11. General Conditions

Sponsor reserves the right to cancel, suspend, or modify the giveaway if fraud, technical failures, or any other factor beyond Sponsor’s reasonable control impairs the integrity of the giveaway, as determined by Sponsor in its sole discretion. Sponsor reserves the right to disqualify any individual who tampers with the entry process or violates these rules.

 12. Privacy

Any personal information collected through the giveaway will be subject to Sponsor’s Privacy Policy which is incorporated into these Official Rules.
